Applications can be submitted in either of the following manner:

You may download the application form as a PDF, fill it out electronically and send it as an attachment with your supporting documents to info@sullivanrenaissance.org. Please note: You need the latest version of Adobe Reader to fill out the form electronically and save it to your computer.
You may download and print the application and fax it with your supporting documentation to 845-295-2746 or mail it to Sullivan Renaissance, Gerry Foundation, PO Box 311, Liberty, NY 12754
If you are unable to print or save the PDF application from the website, please call the Sullivan Renaissance office at 845-295-2445 to have one mailed to you. Microsoft Word versions of the application are also available upon request. Complete and mail back with your supporting documentation.
